Kaiser Aluminum faces a steep fine after a worker there was killed last year.
James Van Doren died in October.
He was crushed while working on a loader at the Mead Aluminum plant.
The Department of Labor and Industries has fined Kaiser $35,000 for five violations.
Kaiser now has 15 days to appeal this citation.
