...to the physical causes of impotence. While alcohol can make you feel virile,
it can also interfere with your ability to sustain an erection.
And the same goes for many recreational drugs.

Well-known DJ, Donnie Sutherland, has experienced the stress of being expected to perform.
On some occasions, I just didn't want to get it up. I didn't think I could get it up.
I just, you know, brushed it under the carpet and said,
oh, you know, like the old headache thing from the wife, you know,
sorry, not tonight, don't have a headache.
The catch with psychological impotence is that when a man has failed to gain an erection several
times, he starts to avoid sex to save himself further embarrassment. And so the problem gets
worse. Of all the things we do in life in general, the harder we try, the better we do,
except for sex. The harder you try, the worse you do.

The solution in Donnie's case was fairly simple. Reassurance by a doctor that he was quite normal
took away much of the stress and anxiety.
Up there, the doctor said, after doing tests and everything, said,
for a fellow 50 years of age, you know, you're quite normal. And really, when I look back at it,
it was psychological only. And it was only in my mind that the worry was there. The nagging doubt
was there and it took going to the doctors to actually, you know, clear my brain and say,
hey, everything's all right.
Physical treatment for impotence has been revolutionized over the last 10 years
with injections into the penis.
It's a lot easier than actually injecting into your arm or your leg,
because you're not injecting into a muscle. It's quick, it's painless, less than 10 seconds.
Side effects include scarring and infection at the injection site,
and on occasion, an embarrassing erection that can last for hours.
Other treatments include vacuum pumps, and in a few extreme cases,
penile rods, which produce an erection when the rods are pumped up.
The penis becomes erect when the man pumps up the balls, which are held inside his testicles.

After speaking publicly, Donny found he'd touched a nerve.
I get people coming up to me saying, you know, that thing you did on the television,
you know, the pump up thing and the needle, and I say, but no, I never did have to use any of those.
But at the same time, when you tell them that these things do work, and or if it's just purely
in the brain, go and see a doctor and it'll help you, you know, sort of revitalise you and give you
a bit of confidence again, they go, oh, thanks very much. And they're really, really happy. So
I don't think that it hurts to talk about it. And I think men should talk about it more.
David was so affected by his impotence, and so impressed with the solution,

when the engines fight up. Likewise, if you want to have an amazing orgasm, the first step is to
learn just what happens to the body when it's being aroused. Well, Dr. Cindy Pan can tell you.
Humans are thought to be unique in their enjoyment of sexual intercourse. And at the peak of that
pleasure, when the body can sustain the build-up of sexual energy no more, we orgasm.
A man's orgasm is obvious externally, but this rare footage shows the female orgasm internally.
A miniature camera has captured the moment of the male orgasm as the penis ejaculates into the
vaginal cavity. In the next shot, the female reaches orgasm and her cervix pulsates and pushes
down towards the pool of semen. And while there's an obvious reason why men orgasm, exactly why women
do isn't clear. Maybe an orgasm increases the probability of a fertilization. I mean, if you've
had sex, if there is seminal fluid inside the vagina, maybe having an orgasm increases the
probability of a sperm and an ovum getting together. But that has not been proved. So we
really don't know, except that it's a great thing to have. For men, uninterrupted sexual activity
almost always leads to ejaculation and orgasm. But for women, things are a little bit more complex.
In fact, only about 50% of women have regular orgasms. But rather than there being any physical
problems, barriers are usually psychological or emotional. So understanding what's happening to
your body as you're becoming aroused is one of the first steps to overcoming difficulties.
Although men and women go through similar arousal stages, what triggers us sexually can vary.
Men can become aroused just by sight and mainly feel sensations in the genital area.
Women, however, usually take longer to reach such a high state of arousal. And they're more turned
on by touch, by feeling nurtured by their partner. Women also experience tingling sensations all over
their body, not just in the genital region. Some people talk about the sound of Mozart's horn
concerto. Mozart's horn concerto has been sensually arousing for them. But for someone else,
it's deep breathing that is very arousing. So when we talk about our senses, every sense from
touch, and we talk about sadomasochism, that's pain. That's something that people find sexually
arousing. So anything can be sexually arousing depending on what that person has been,
almost culturally, socially brought up to regard as sexually arousing or acceptable
as a sexually arousing stimulus. In response to these arousal triggers, the body releases hormones
like adrenaline, which in both sexes leads to a faster heart rate, rapid breathing and a rise in
blood pressure. As feelings intensify, so do physical changes. And this stage is aptly known
as the excitement phase. In men, the most obvious manifestation of sexual excitement is an erect
penis caused by the engorgement of penile tissue with blood. The blood is trapped in the penis,
which grows and becomes rigid. To move from arousal to orgasm, the penis needs to be
continuously stimulated. This is usually done by hand or mouth or by movement in a vagina or anus.
In women, the excitement stage sees the vagina moistened and lubricated with fluid.
Clitoral orgasms are achieved by continued stimulation of the clitoris,
usually by hand, mouth or penis. Many women orgasm this way because the clitoris is their
most sensitive sexual organ. When touched, like the penis, it becomes erect. Some women also have
vaginal orgasms. These are achieved by deep thrusting into the vagina, usually by a penis.
When a woman is sexually aroused, this whole area, which we call the pelvis,
becomes congested with blood, gets swollen, congested with blood. And that is the basic
localized response to sexual arousal. Extra blood flow also swells the inner lips,
stretching them away from the opening. This, as well as the vaginal lubrication,
prepares a woman's vagina for penetration. Both sexes experience a flushing across the chest
and erect nipples. From the excitement phase, our bodies move into the plateau phase. This stage
lasts anything from a few seconds to a few minutes. And far from the situation leveling out,
as the word suggests, it moves us rapidly closer to orgasm. For men at plateau stage,
erection is complete. The testes draw tighter to the body and increase in size. Continued stimulation
of the penis nearly always leads to orgasm and ejaculation. Nerve impulses from the spinal cord
cause rhythmic, wave-like contractions of the pelvic floor muscles. And this movement propels
semen from the testes and out of the penis. For women, continued stimulation means the clitoris
has retracted under its hood and there is a further widening of the back two-thirds of the vagina.
Like males, the female orgasm involves rhythmic contractions in the pelvic region.
There are usually five to eight spasms, about 0.8 of a second apart. But every woman is different.
Some have the capacity to orgasm for up to 50 seconds, whereas others don't orgasm at all.
In an orgasm in a female, it's mainly a cerebral thing. See, they don't have to produce an ejaculate.
It's mainly a cerebral thing. So when they are tensed up, you know, they're trying to get an
orgasm. That is probably the best way of inhibiting it. Our society often places great importance on
the need to have orgasm, what they should feel like and what they should be. The truth is,
all our orgasmic experiences vary enormously and the key to unlocking them is to block out
any external pressures and allow our sensations to take over. In the 90s, young people seem happy
to discuss orgasms, sex and sexuality. But few older people will tell the world about their sex
lives until now. Do you really remember the first time? Yes.
You never really told me what you thought of it. You told me the third time, not the first.
Talk about that sort of thing. That was... But they talk about it all the time now,
so why can't we talk about it in retrospect, as it was? Oh, in retrospect, it's different.

you reach the peak. And orgasms feel different every time you have one. Sex therapist Jan Hulse
says that if you want to experience a whole range of orgasmic bliss, it's time to discover
how to tap into the many different types of orgasm. Most people know about clitoral or
vaginal orgasms, but there's a much wider range of orgasms to choose from than those
for both partners. For instance, there's the orgasm you can have once you find your G-spot.
For the male, it's actually the prostate gland is the main functioning spot for that kind of
arousal. And the woman can find it by going for that spot between the scrotum and the anus,
that sort of flat spot. And if you press hard there, then the man can get an intense feeling
of pleasure. Now, for those who are really game, or if you're wearing rubber gloves,
you can actually access the prostate glands through the anus. The female G-spot is to be
found about five centimetres inside the vagina. And you find it mostly by probing. It's about
as big as a 10 cent piece. And it's more spongy, tough kind of feel instead of the normal shape of
the vagina walls. Now, some women say that the first sensation that they get is a feeling that
they want to perhaps, you know, urinate, go to the toilet, because it's very close to the bladder.
But if you persist past that feeling, you'll start to get a very pleasurable sensation.
A less well-known type of orgasm is the exercise orgasm.
Now, exercise orgasms are sometimes very embarrassing if they happen in the middle of the gym,
and perhaps you're not quite ready for them. But it's got to do with those pubococcygeal
muscles, which are deep down. And in the gym, you're usually busy, you know, contracting your
stomach muscles, tensing your legs, perhaps tensing your whole torso, and so they can sneak up on you.
There are different styles of orgasm, and women can have many orgasms during lovemaking.
A lot of women have experienced multiple orgasms through lovemaking, and that is particularly
through the clitoral kind of stimulation, so that they can actually reach a peak,
and then it will die down. And then with further stimulation, particularly with perhaps swapping
from maybe manual stimulation, then to an oral sensation, they will experience another peak
and another clitoral orgasm muscular release. And then perhaps through intercourse,
they can experience yet again another clitoral orgasm.
There's another kind of orgasm that women can train themselves to experience,
and that's a prolonged multiple orgasm. That is where they'll, after the first
clitoral orgasm, then with the G-spot stimulation, they can actually reach a peak where it's actually
firing off orgasmic contractions virtually every minute, and this can go on and on and on.
Now, a prerequisite for women to have prolonged multiple orgasms, it's a pre-requisites,
is that they have learned to masturbate themselves to an orgasm at least three times
within 15 minutes. And when trying to achieve prolonged multiple orgasms,
it's important to learn how to relax and receive. What I'd recommend is that as you're learning how
to stimulate, especially the G-spots, the ideal scene is for the first hour the woman just receives,
and you can do this perhaps by lying down on your tummy first of all, with the man coming in from
underneath, and you must relax and you must allow inside of your head for yourself to receive.
And then it's time to let the man spend an hour of receiving.
It's important that he try to maintain his erection for that hour.
Stage two is doing a lot more oral and manual stimulation perhaps for about the same time,
and the theory is if you're willing to go through these two stages,
then when you're finally willing to have intercourse, you'll actually find the man's
naturally going to prolong his erection much longer, and he'll also have a bigger release of
semen. And most guys will acknowledge that his most powerful orgasms come from when he's had
the biggest release of sperm. Men are capable of having a number of
orgasms during lovemaking because it's possible to orgasm without ejaculation.
I think that there are a lot of men out there who just have no idea about this possibility,
that if it's not only prolonging the actual perhaps intercourse or foreplay and erect penis,
it's actually being able to be aroused to the point it's just prior to ejaculation where you
are experiencing that intense pleasure and that muscular contractions that's spontaneous and
uncontrollable, but it's actually before the ejaculation.
If you can have a goal of we are together in this, we can pleasure each other,
and we can teach ourselves and each other to have multiple continuous prolonged orgasm,
you are extending the likelihood that your lovemaking will be tender,
it'll be joyous, and it will be the most intensely pleasurable that you possibly can have.
